Blunt Trauma is an achievement/trophy available in Escape Dead Island. This will automatically unlock as you progress through the game. To get this you need to pick up the makeshift club, that you get at the start of Mission 4 - Marathon from inside the shipping container that Cliff wakes up in. The club is on a table to go over to it and pick it up. This will unlock when you do.
